 Welcome to our website and our practice! We want you and your pet to be comfortable and to feel well attended and cared for when you visit us. To make things run more smoothly for your first visit, we usually ask you some basic information when you call for an appointment. First we will need your name and address and a telephone number. Next we will need to know some details about your pet such as his or her name, date of birth or age, breed, color, and sex. If you have any previous medical records or information from a breeder we would like to include a copy of these in your pet's new record. We can copy these when you arrive if you have them, or we can call your previous veterinarian for you and have these faxed or e-mailed in time for your visit with us.
If you would like to fill out a new client - new patient form here online before your visit and e-mail it to us, please let us know when you call to make your pet's first appointment. That way we will be expecting it and can make a new computer medical record and a 'hard copy' record for your pet based on the information you provide.
